Documents Required:
1. Canadian Pension Plan (CPP)
2. Old Age Security (OAS)
3. Public Service Alliance of Canada (PSAC)
4. The Ontario Municipal Employees Retirement System (OMERS)
5. Personal Bank Statements

Special Guidelines:
1. Bank Statements must cover the last 3 months. Documents covering other periods of time will not be accepted.
2. If not providing actual statements issued by your financial institution, please ensure that the account holder's name is visible.
3. CPP, OAS, PSAC and OMERS must be of the most recent year.
